Conference Cancún Presenter Conflict of Interest Form

Each author of each presentation at Conference Cancún must complete and submit a Conference Cancún Presenter Conflict of Interest form. 

How to complete and submit:

• download the form fillable document 
• save it on your local computer
• open it in Adobe, either Reader or the full writer product. Do not use your web browser to enter your information.
• complete and save the filled document 
• upload the completed form into the Google Drive folder that has been designated for your submission id number, which you will have received via email from ISAAC.
